PRESCRIPTION FOR TREATMENT
Most patients come to us with a prescription for physical therapy that is issued by their physician and includes recommendations for treatment. You may see a Physical Therapist at IslandFit physical therapy without a referral or prescription, however, continuation of treatment without a referral is dependant on your insurance plan.

What to wear
We usually recommend that patients wear loose-fitting attire or exercise clothing and sneakers for maximum comfort.
What to bring
- Insurance Cards
- Physical Therapy prescription/referral from a physician
- Picture ID
- Medication List, or Operative reports
- Reports from any diagnostic tests (For example: X-rays, MRI, EMG)
- Comfortable and loose fitting clothing and exercise shoes
- Any braces, prosthesis, or assistive devices you are using
- Downloaded and filled out new patient forms
